Job summary

Alternate Solutions Health Network seeks a Physical Therapist Assistant (PTA) to provide skilled services in patients’ homes under a Physical Therapist's direction. You will educate patients and families, ensure safety, and complete compliant documentation.

The role requires at least 1 year of PTA experience in acute settings, preferably with home care exposure; candidates must have a valid driver’s license and auto insurance and demonstrate strong communication and clinical judgment.

Qualifications

• Minimum of one year of PTA experience in an acute setting.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Ability to identify situations and apply best solutions.
• Detail-oriented and observant.
• Disciplined work ethic with ability to prioritize and be timely.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provides skilled PTA services in a patient’s home as directed by the Physical Therapist plan of care.
• Observes, records, and reports patient reactions, changes, and progress.
• Performs physical therapy procedures and diagnostic tests as prescribed.
• Educates patients and families on therapy techniques, safety measures, and home exercises.
• Completes documentation per Medicare/Federal guidelines and agency protocol.
• Drives to patient locations and maintains daily visit schedules.
• Communicates with scheduler and care team; participates in case conferences and on-call rotations.
