Job summary

Alternate Solutions Health Network, LLC is seeking a licensed Physical Therapist Assistant (PTA) to provide skilled services in patients’ homes under the direction of a Physical Therapist. The PTA educates patients and families, ensures safety, and maintains compliant documentation per Medicare and company policy.

The PTA I will travel to patients’ primary locations, document visits within 24 hours, and participate in on-call case conferences.

Qualifications

• One year PTA experience in an acute setting required.
• Current PTA license for the state of employment.
• Excellent communication with patients, families, physicians and coworkers.
• Knowledgeable on therapy best practices.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled services in patients’ homes per Physical Therapist plan of care.
• Observe, record, and report patient progress and reactions.
• Perform PT procedures and diagnostic tests as prescribed.
• Educate patients and families on therapy techniques and safety.
• Document treatment, assessments and education in a timely manner.
• Coordinate with therapists and other staff to promote coordinated care.
• Attend case conferences and on-call requirements as needed.
• Drive to patient locations and document visits within required timelines.
