Job summary

Alternate Solutions Health Network is seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled services in patients' homes under the supervision of a Physical Therapist. You will educate patients and families, ensure safety, and maintain compliant documentation per Medicare and agency guidelines.

The role requires a current PTA license in the state of employment and at least one year of acute care PTA experience.

Qualifications

• PTA with current license in the state of employment.
• Minimum of one year of PTA experience in an acute setting.
• Home care experience is desirable.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledge of therapy best practices.
• Ability to make appropriate physical therapy judgments.
• Detail-oriented and observant.

Responsibilities

• Provides skilled services in the patient’s home as developed by the Physical Therapist plan of care.
• Observes, records, and reports patient reactions, changes, and progress.
• Performs physical therapy procedures and diagnostic tests as prescribed.
• Treats patients to relieve pain and restore function.
• Assists in achieving goals to improve patient functional status.
