Eventus WholeHealth is seeking a part-time psychotherapist (LCSW/LISW/LPCC/LCMHC/LMFT) to provide recovery-oriented psychotherapy in skilled nursing and assisted living facilities. The clinician will work within a multidisciplinary team to assess residents, develop treatment plans, and deliver care in both group and individual settings.

Qualifications

Responsibilities

• Document treatment plans and sessions in EMR within required timelines.
• Coordinate with facility staff and primary care providers.
