Qualifications

• Requires strong assessment and treatment planning abilities.
• Experience with group or individual psychotherapy in long-term care settings preferred.
• Must be licensed as LCSW/LISW/LPCC/LCMHC/LMFT in the state where practicing.

Responsibilities

• Assess residents with consent and develop individualized treatment plans.
• Coordinate with primary care teams and facility staff.
• Document treatment plans and sessions per policy within 48 hours.
• Maintain HIPAA-compliant confidentiality and professional conduct.
• Collaborate with clinical supervisor on complex cases.
