Job summary

Eventus WholeHealth delivers an integrated model of care to residents in skilled nursing and assisted living facilities. We seek a licensed clinician (LCSW/LISW/LPCC/LCMHC/LMFT) to provide recovery-oriented services in group or individual settings as part of a multidisciplinary team.

You will assess, plan, and implement care for residents with complex mental health needs while maintaining confidentiality and professional ethics in accordance with HIPAA and facility policies.

Qualifications

• Master’s degree in social work and current state license.
• Licensure options include LCSW/LISW/LPCC/LCMHC/LMFT depending on state.
• Experience in mental health therapy within long-term care is preferred.

Responsibilities

• Assess residents with consent and develop individualized treatment plans.
• Consult with primary care physicians, family, facility staff as needed.
• Document treatment plans and psychotherapy sessions; provide written updates within 48 hours.
• Coordinate with Resident Care Coordinator or staff to review facility care needs.
• Maintain open, professional communication with facility staff and administration; share care information as required.
• Adhere to Eventus WholeHealth policies, HIPAA, and regulatory guidelines.
• Act professionally and protect patient confidentiality at all times.
• Consult with clinical supervisor when faced with unfamiliar resident or requests.
