Continuum Therapy Partners, LLC is seeking a Physical Therapist in LaBelle, FL. The role emphasizes patient outcomes by listening to therapists and aligning with leadership that prioritizes clinicians. The position is full-time with daytime hours at LaBelle Health and Rehabilitation Center.

Qualified candidates hold a DPT, an active PT license in Florida (NPTE) and ideally 1 year of experience. They will develop and adjust individualized treatment plans, document progress, and educate patients

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; NPTE required.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
