Continuum Therapy Partners is seeking a Physical Therapist for LaBelle Health and Rehabilitation Center in LaBelle, FL. The role focuses on evaluating patients, developing individualized plans in coordination with other disciplines, and documenting outcomes to guide care.

You must have a DPT, an active state license, and NPTE must be passed; one year of experience is preferred. This is a full-time daytime position with comprehensive benefits and licensure reimbursement.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
