Continuum Therapy Partners is seeking a Physical Therapist to join our team at Northern Lights Services in Washburn, WI. We value clinicians' voices and patient-centered care, offering a supportive environment through a full-time daytime schedule.

The ideal candidate will hold a Doctor of Physical Therapy (DPT) degree, be licensed in Wisconsin, pass the NPTE, and have around 1 year of experience; they will develop and implement individualized plans of care and educate patients and families.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred
