Job summary

Continuum Therapy Partners is seeking a licensed Physical Therapist in Northfield, MN for a full-time daytime role. The candidate will evaluate patients, develop and implement individualized treatment plans, and coordinate with other disciplines to optimize outcomes.

Responsibilities include documenting progress, adjusting plans based on outcomes, and educating patients and families on home exercise programs and injury prevention.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
