Continuum Therapy Partners is seeking a Physical Therapist to join our network of facilities with 275 locations across 16 states, including Effingham, IL and Olney, IL. This full-time daytime role focuses on developing and implementing individualized treatment plans in collaboration with a multidisciplinary team.

The ideal candidate holds a Doctor of Physical Therapy, active state license, NPTE eligibility, and prefers at least one year of PT experience.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE eligibility.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

This is a split position covering Effingham & Olney, IL

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
