Continuum Therapy Partners in Minnesota is seeking a Doctor of Physical Therapy to join our growing network at Whitewater Health Services in St. Charles.

You will evaluate, develop, and implement individualized treatment plans in coordination with other disciplines, document progress, and educate patients and families on home programs. Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• 1 year of prior PT experience preferred.

• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
