Job summary

Continuum Therapy Partners seeks a Physical Therapist to join our Illinois network. You will evaluate and treat patients, develop individualized plans of care, and coordinate with other disciplines to improve outcomes.

The role is full-time with daytime hours, based at Taylorville Care Center and Montgomery Nursing & Rehab Center in Taylorville and Hillsboro. A DPT, current PT license, and NPTE completion are required; 1 year of experience preferred.

Qualifications

• DPT degree required.
• Current, unencumbered PT license in the state.
• Must pass the NPTE.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
