Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist in Olney, IL to evaluate and treat patients, develop personalized plans, and coordinate with care teams for optimal outcomes.

Requirements include a Doctor of Physical Therapy (DPT), current IL license (NPTE), and about 1 year of PT experience preferred, with a daytime PRN schedule and a supportive benefits package.

Physical Therapist Requirements & Responsibilities

• ?Doctor of Physical Therapy (DPT) degree required


• ?Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)


• ?1 year of previous experience as a Physical Therapist preferred


• ?Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines


• ?Documents patient progress and adjusts plans of care based on clinical outcomes


• ?Educates patients and families on home exercise programs and injury prevention
