Job summary

Continuum Therapy Partners, LLC is seeking a dedicated Physical Therapist to join our network of care. This split position covers locations in IL and KS, with a primary placement at Delmar Gardens of Overland Park in Overland Park, KS, offering a comprehensive benefits package.

You will evaluate and implement individualized treatment plans, document patient progress, and educate patients and families on home exercise programs and injury prevention, in collaboration with a multidisciplinary team.

Qualifications

• DPT degree is required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
