Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to split duties between Effingham and Olney, IL. You will evaluate and implement individualized treatment plans, coordinate care with other disciplines, document progress, and educate patients and families on home exercises and injury prevention.

The role is full-time with daytime hours, located at Evergreen Nursing & Rehab in Effingham and Helia Healthcare of Olney, IL.

Qualifications

• Doctor of Physical Therapy (DPT) degree is required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
