Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our growing network at Evergreen Nursing & Rehab in Effingham, IL. The role focuses on evaluating patients, developing individualized treatment plans, and coordinating care with other disciplines to improve outcomes.

The ideal candidate holds a Doctor of Physical Therapy (DPT) degree, an active IL license, and NPTE certification. One year of PT experience is preferred; PRN daytime coverage is available.

Qualifications

• DPT degree is required for this role.
• Active state license in IL and NPTE certification required.
• 1 year of PT experience preferred.
• Develop and implement individualized treatment plans in coordination with other disciplines.

Responsibilities

Physical Therapist Requirements & Responsibilities

• ?Doctor of Physical Therapy (DPT) degree required
• ?Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• ?1 year of previous experience as a Physical Therapist preferred
• ?Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• ?Documents patient progress and adjusts plans of care based on clinical outcomes
• ?Educates patients and families on home exercise programs and injury prevention
