Job summary

Continuum Therapy Partners in Illinois is seeking a licensed Physical Therapist to join our growing network across facilities. You will evaluate and implement personalized treatment plans, collaborate with a multidisciplinary team, and contribute to high-quality patient outcomes.

Requirements include a Doctor of Physical Therapy, an active IL license (NPTE eligible), and a year of experience preferred; we offer a PRN daytime schedule, 401k with match, and generous PTO.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
