Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to support patient outcomes at Aviston, IL. A Doctor of Physical Therapy (DPT) is required, with an active IL license and NPTE eligibility.

Prior PT experience is preferred, and you will evaluate and implement individualized treatment plans in coordination with other disciplines. The role includes documenting patient progress, adjusting plans of care based on outcomes, and educating patients and families on home exercise programs

Qualifications

• DPT required.
• Current, unencumbered state PT license; NPTE required.
• 1 year of PT experience preferred.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
