Job summary

Evolve Therapy Services in Oklahoma City, OK is seeking a dedicated Physical Therapy Assistant to join our in-house skilled nursing facility team. You will provide therapy services under the supervision of a Physical Therapist and help implement treatment plans to promote resident mobility and independence.

The position supports scheduling, screenings, and documentation in NetHealth EMR, with opportunities for ongoing education and growth.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and development of treatment plans with the Physical Therapist.
• Assist in establishing and modifying resident schedules with the Physical Therapist and team.
• Train residents and staff in use of equipment and modalities.
• Ensure compliant and medically necessary documentation in NetHealth EMR.
