Job summary

Evolve Therapy Services in Temple, OK seeks a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. This role supports the Physical Therapist, helping deliver care and monitor residents' progress in a collaborative team setting.

Full Time, Part Time, or PRN opportunities; duties include screenings, assisting in evaluations, implementing treatment plans, and educating residents and staff. A license and ongoing education are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules in conjunction with the Physical Therapist and other team members.
• Conduct both individual and group treatment including strength, mobility, balance, gait, ROM activities and modalities.
• Monitor residents' response to intervention and consults with the Physical Therapist if modification is indicated.
• Train resident, family and appropriate staff in the use or application of equipment.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.

Education

Associate's degree in physical therapy from an accredited program.

Active Physical Therapy Assistant license in state(s) of practice.

Ongoing continuing education and professional development to maintain current licensure and certification.
