Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Temple, OK. You will work under the supervision of a Physical Therapist to deliver patient-centered therapy, evaluate needs, and help implement individualized treatment plans.

Responsibilities include performing both individual and group therapy sessions, monitoring progress, documenting CMS-compliant notes in NetHealth, and collaborating with the PT and care team to

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.
• Document compliant CMS records in NetHealth EMR.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in establishing treatment plans with goals.
• Carry out individual and group therapies addressing strength, mobility, balance, gait, and ROM.
• Monitor resident response and report modifications to the PT.
• Maintain resident schedules and participate in QA programs.
