Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Reading, MA. The role involves providing therapy under the supervision of a PT, conducting screenings, and assisting in evaluations to support resident care plans.

The position offers full-time or PRN options with benefits and opportunities for growth within the organization. Experience with NetHealth EMR and CMS documentation is preferred.

Qualifications

• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Associate's degree in physical therapy from an accredited program.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the PT and team.
• Document therapy services and progress in NetHealth EMR as required by CMS.
• Train resident, family and staff in use of equipment and treatment modalities.
• Participate in resident, family, and staff education.
• Ensure treatment plans align with federal, state, and local standards.
