Job summary

Palms Home Care is seeking a skilled Home Health Physical Therapist (PT) to evaluate and treat patients in their homes, promoting mobility and independence. You will design individualized plans, educate families, and work with a compassionate interdisciplinary team to ensure the highest standard of care.

The role requires a Florida PT license, CPR certification, and preferred 1–2 years in Home Health. A valid driver’s license and reliable transportation are essential to visit patient homes and

Qualifications

• Current Physical Therapist license in Florida.
• Graduate of an accredited PT program.
• 1–2 years of Home Health experience preferred.
• Strong communication skills.
• Ability to work independently.
• CPR certification (current).
• Valid driver's license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans to improve mobility and independence.
• Provide therapeutic exercises, manual therapy, and mobility training in patients' homes.
• Assess patient progress and adjust care plans accordingly.
• Educate patients and caregivers on safety and exercises.
• Maintain timely and accurate documentation of patient care.
• Collaborate with interdisciplinary teams and adhere to home health and Medicare guidelines.
