Job summary

Palms-Home-Care in Orlando, FL is seeking a skilled Home Health Physical Therapist (PT) to provide individualized in-home physical therapy services, helping patients improve mobility and regain independence after illness or surgery.

The successful candidate will perform initial evaluations, develop treatment plans, and educate patients and caregivers on safety and exercises while coordinating with interdisciplinary care teams and adhering to Florida AHCA guidelines.

Qualifications

• Current Physical Therapist license.
• Graduate of an accredited PT program.
• CPR certification current; valid driver’s license and reliable transportation.
• Knowledge of OASIS is a plus.
• Experience with EMR systems such as Kinnser.

Responsibilities
