Job summary

Diamond Care Group in the United States is seeking a Home Health Physical Therapist (PT) to deliver patient-centered physical therapy in patients' homes, helping them regain mobility and independence after illness, injury, or surgery.

You will perform initial evaluations, develop individualized treatment plans, provide therapeutic exercises and manual therapy, educate patients and caregivers, document thoroughly, and collaborate with interdisciplinary teams while following home health and

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1–2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains current CPR certification.
• Valid driver's license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
