Job summary

Palms Home Care is seeking a skilled Home Health Physical Therapist to evaluate patients, design individualized treatment plans, and guide recovery in the home setting. You will perform therapeutic exercises, manual therapy, and mobility training, while educating patients and families on safety and exercises.

The role requires a licensed PT with Home Health experience, strong communication, and the ability to work independently. A valid driver's license and CPR certification are essential.

Qualifications

Key Responsibilities:

• Perform initial evaluations and develop individualized treatment plans to help patients improve mobility and regain independence
• Provide therapeutic exercises, manual therapy, and mobility training to patients in their homes
• Assess patient progress and adjust care plans accordingly
• Educate patients and caregivers on safety and exercises
• Maintain timely and accurate documentation of patient care
• Collaborate with interdisciplinary care teams and adhere to home health and Medicare guidelines

Qualifications:

• Current Physical Therapist license
• Graduate of accredited PT program
• 1-2 years of experience preferred in Home Health
• Strong communication skills
• Ability to work independently
• Possesses and maintains current CPR certification
• Valid driver's license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser
