Job summary

Palms-Home-Care in Port Charlotte, FL is seeking a dedicated Home Health Physical Therapist to provide in-home clinical services, improve mobility, and help patients regain independence after illness, injury, or surgery.

You will perform initial evaluations, develop individualized plans, educate patients and caregivers, and collaborate with interdisciplinary teams while adhering to Florida AHCA/home health guidelines and using EMR systems such as Kinnser.

Qualifications

• Current Physical Therapist license or equivalent.
• Graduate of an accredited PT program.
• 1–2 years of Home Health experience preferred.
• Strong communication skills and ability to work independently.

Job Summary

Our company is expanding and we are seeking a skilled Home Health Physical Therapist (PT) to join our team. In this role, you will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence following illness, injury, or surgery.

• Experience with EMR systems such as Kinnser
