Job summary

Palmshomecare is seeking a skilled Home Health Physical Therapist (PT) to join our team. You will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence after illness or surgery.

Responsibilities include evaluations, treatment plans, therapeutic exercises, progress assessments, and caregiver education, while maintaining documentation and adhering to home health and Medicare guidelines.

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1-2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains current CPR certification.
• Valid driver's license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
