Job summary

Palms-Home-Care is seeking a skilled Home Health Physical Therapist (PT) to join our team. You will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence after illness, injury, or surgery.

Responsibilities include evaluating patients, creating treatment plans, guiding exercises, and coordinating with interdisciplinary teams while complying with Home Health and Medicare guidelines.

Qualifications

• 1–2 years of experience preferred in Home Health.
• Must hold a valid PT license and be a graduate of an accredited PT program.
• CPR certification must be current.
