Job summary

Diamond Care Group is seeking a skilled Home Health Physical Therapist (PT) to join our team. You will provide individualized physical therapy services to patients in their homes, helping them regain mobility and independence after illness, injury, or surgery.

Responsibilities include conducting evaluations, creating treatment plans, and educating patients and caregivers. Strong communication and ability to work independently are essential, with credentials in PT and CPR current.

Qualifications

• Current Physical Therapist license
• Strong communication skills
• 1-2 years of Home Health experience preferred
• CPR certification current
• Valid driver’s license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser
