Job summary

Palms-Home-Care in Sarasota, FL is seeking a skilled Home Health Physical Therapist to join our team. You will provide individualized therapy to patients in their homes, helping them improve mobility and regain independence after illness, injury, or surgery.

You will perform evaluations, develop treatment plans, guide patients and caregivers, and document progress in our EMR. The role requires Florida licensure, CPR, and a valid driver's license with reliable transportation.

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1-2 years of Home Health experience preferred.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform initial evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, manual therapy, and mobility training.
