Responsibilities

• Lead provider performance programs evaluated by metrics (4 STAR gap closure, coding accuracy).
• Build relationships with care providers and provide education and reporting.
• Identify performance improvement opportunities through analysis with SMEs.
• Coordinate provider incentive programs for the health plan.
• Develop provider-specific plans to improve HEDIS performance and outcomes.
• Provide training and coaching on program implementation and barrier resolution.
• Lead internal resources for provider education and consultation, including Stars measures.
