Job summary

UnitedHealth Group is seeking an Associate Practice Performance Manager to implement programs and manage provider performance across Medicare Advantage plans. You will review charts (paper and EMR), identify STAR opportunities, coordinate preventive care, and guide care coordination efforts.

Expect extensive collaboration with physicians and medical groups, plus education and reporting to drive improvements.

Qualifications

• 3+ years of healthcare industry experience.
• 1+ year of experience working for a health plan and/or a provider's office.
• Proven solid communication and presentation skills.
• Proven solid relationship building skills with clinical and non-clinical personnel.

Responsibilities

• Meet with providers to discuss Optum tools and programs to improve care quality.
• Assist in reviewing medical records to highlight STAR opportunities.
• Handle data collection, data entry, quality monitoring and chart collection activities.
• Identify gaps in care and coordinate preventive screenings and care management.
• Develop provider-specific plans to improve HEDIS performance and outcomes.
• Provide training and coaching on program implementation and barrier resolution.
• Lead monthly provider meetings to drive process improvements.
