UnitedHealthcare is seeking a Practice Performance Manager to lead provider performance initiatives within Medicare Advantage programs. The role focuses on 4 STAR gap closure, coding accuracy, and proactive performance improvement through education, reporting, and collaboration with care providers.

Travel up to 75% within the Ft. Myers, FL territory to meet providers, implement Stars measures, and drive program adoption with physicians, IPAs, and ACOs.

Qualifications

• 5+ years of healthcare industry experience.
• 1+ year of STARs experience.
• Driver's License and access to reliable transportation.
• Live within or in close proximity to Ft. Myers, FL territory.

Responsabilités

• Manage provider performance using designated metrics (4 STAR gap closure, coding accuracy).
• Build relationships with care providers and deliver education and reporting.
• Travel across assigned territory to discuss UHG tools and programs with providers.
• Execute provider incentive programs for the health plan.
• Establish consultative relationships with physicians, medical groups, IPAs and ACOs.
• Develop provider-specific plans to increase HEDIS performance and outcomes.
• Provide training and coaching on program implementation and barriers.
• Coordinate resources for provider education and Stars measures training.
• Lead Stars-focused meetings to drive continuous improvement and goals.
• Provide progress reporting to health plan leadership on performance and gap closures.
• Facilitate monthly/quarterly meetings and prepare materials.
• Offer feedback to Optum and the health plan.
