Job summary

Strive Health Services LLC is seeking a Network Engagement Partner to maintain external provider relationships in markets where Strive is contracted. You will establish and grow provider networks to ensure high-quality, continuous care for CKD and ESRD patients.

You will travel to meet stakeholders, analyze utilization data, and collaborate with market directors to shape outreach strategies and improve clinical outcomes while supporting new workflows and tools.

Qualifications

• High school diploma or GED equivalent.
• 6+ years combined of related education, experience, or certification.
• Efficient and reliable transportation, including an active driver's license.
• Ability to travel and be on-site to meet business needs.
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down).

Responsibilities

• Maintains consistent and strong relationships with provider groups using an account management approach.
• Analyzes utilization, claims, demographics to outline network strategy with market directors.
• Uses a data driven approach to maintain relationships and reduce total cost of care.
• Reviews data and reporting with provider groups to improve outcomes.
• Collaborates with business development to identify partnership opportunities.
• Drives performance of portfolios with engagement and quality targets.
• Engage, contract, and manage practices in incentive programs.
