Responsibilities

• Develop, prepare, and monitor individual support plans according to guidelines.
• Coordinate appointments, meetings and administrative matters for individuals served.
• Assist individuals and families, and collaborate with agencies.
• Coordinate admissions paperwork and discharges as needed.
• Conduct unannounced visits as needed.
• Ensure paperwork is complete and submitted by due dates.
• Assist Executive Director with program expansion and service development.
• Promote community acceptance and liaison with other agencies.
• Adhere to safety, quality and corporate policies.
