Job summary

BCFS Health and Human Services is seeking a Program Supervisor to directly supervise the Lead Case Manager, Case Managers, and Community Health Workers, ensuring quality assurance and improvement through home visits, productivity meetings, and chart audits.

The role requires a Bachelor’s degree and 2 years of supervisory experience, with flexibility to work evenings, weekends, and holidays as needed to support program operations.

Qualifications

• Bachelor’s degree from an accredited program/university in a related field.
• 2 years of supervisory experience.
• Maintain certificates required for the position including First Aid and CPR.
• Valid Texas Driver’s license with a clear driving record and reliable transportation.

Responsibilities

• Directly supervising the Lead Case Manager, Case Managers, and Community Health Workers.
• Oversee home/outreach observation visits, weekly productivity meetings, and chart audits.
• Coordinate supplemental programs and projects and participate in partner agency calls.
• Ensure referrals are based on client histories and needs; conduct health education classes as needed.
