Qualifications

• Bachelor’s degree from an accredited program/university in a related field was required.

Responsibilities

• Supervise Lead Case Manager, Case Managers, and Community Health Workers with QA and improvement activities.
• Oversee intake assessments, benefit assessments/applications, goal setting, and case plan development.
• Lead home visits, monitor progress with standardized tools, and ensure client advocacy and referrals.
• Coordinate supplemental programs, coalition involvement, and innovation networks.
• Attend conference calls with partner agencies as assigned by the program director.
• Ensure referrals are based on client histories and needs.
• Conduct health education classes and document evaluations and data in the database.
• Monitor productivity, provide feedback, and enforce BCFS safety protocols.
• Work evenings, weekends, and holidays as needed; participate in meetings and development activities.
• Maintain professional standards and dress code; promote respectful interactions.
