Job summary

PATH is hiring a Problem-Solving Coordinator in Los Angeles, CA to join the Family Solutions Center. The role focuses on crisis response, rapid resolution, and integrating Problem-Solving throughout the FSC to aid guests with housing goals.

Responsibilities include coordinating with staff, training, referrals, ISP development, HMIS data entry, and collaboration with partner systems, with regular guest meetings and emphasis on ethical practice.

Qualifications

• One year of experience in healthcare, case management, homeless services or related field.
• Ability to analyze data and identify trends for decision making.
• Strong collaboration with colleagues, participants, and stakeholders.
• Clear communication, both written and verbal.
• Creative initiative and proactive problem-solving.
• Maintain ethical standards and professional boundaries.

Responsibilities

• Work with case managers in interim housing settings to support Problem-solving integration.
• Assist with training FSC staff on Problem-Solving.
• Coordinate with LAHSA Problem-Solving Unit for PSAF requests and updates.
• Ensure Problem-Solving tracking across the FSC.
• Process referrals from funders and manage onboarding.
• Meet weekly with guests to support housing service plan progress.
• Develop ISP addressing barriers to services, housing, and health care needs.
• Attend trainings and learning collaboratives as assigned.
• Facilitate psychosocial rehabilitation groups as required.
• Conduct VI-SPDAT at admission for ISP development.
• Enter new guest data into HMIS timely.
• Provide ongoing case management support including benefits and ID/documents.
