Job summary

You will manage housing placement, maintain HMIS/CS Care records within 24 hours, and collaborate with partner agencies to secure permanent housing while ensuring accuracy and HIPAA compliance.

Qualifications

• Some college courses or related administrative experience.
• Minimum two years of job-related experience with homeless individuals and families.
• TB test required every year; CPR and First Aid certification required.
• Able to communicate effectively, both written and oral.

Responsibilities

• Coordinate housing placement and supportive services for families.
• Maintain participant records in HMIS and CS Care within 24 hours.
• Update housing status and case notes promptly as changes occur.
• Prepare weekly/monthly program reports and deliverables.
• Collaborate with agencies (DMH, DPSS, LAUSD) to secure housing.
