Job summary

PATH (People Assisting The Homeless) is hiring a Problem-Solving Coordinator for the Prevention track in Los Angeles, CA. The role focuses on rapid resolution and diversion to access housing resources outside the crisis system.

Responsibilities include collaborating with case managers, training staff, coordinating with LAHSA, and guiding guests through ISP development, benefits, and housing referrals. This is a full-time, non-exempt position with driving required.

Qualifications

• One (1) year of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.

Responsibilities

• Work with case managers in interim housing settings to support the integration of Problem-solving in their cases.
• Assist with training all FSC staff on Problem-Solving.
• Work with the LAHSA Problem-Solving Unit to streamline PSAF requests and intervention updates.
• Ensure Problem-Solving Tracking is happening across the FSC; collaborate with partner family systems to integrate Problem-Solving upstream from Coordinated Entry.
• Process and accept referrals from funders.
• Meet with assigned guests at least weekly to support progress toward Housing Service Plan goals.
• Conduct intake and develop Individualized Service Plans (ISP) addressing barriers to services, housing and health care.
• Attend trainings and learning collaboratives as assigned; facilitate psychosocial rehabilitation groups as required.
• Conduct a full VI-SPDAT at admission to aid ISP development and enter data into HMIS.
• Provide ongoing case management for benefits, IDs, birth certificates, and housing referrals.
