Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Ponca City, OK. This PRN role supports residents under the supervision of a Physical Therapist, conducting screenings and helping develop treatment plans to improve mobility and function.

The successful candidate will hold an Associate's degree in physical therapy, an active PTA license, and be committed to ongoing professional development and CMS-compliant documentation in the NetHealth

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluating residents and developing treatment plans.
• Lead individual and group treatment sessions, including mobility and gait training.
• Document CMS-compliant, medically necessary notes in NetHealth EMR.
