Job summary

Evolve Therapy Services in Jellico, TN seeks a Physical Therapy Assistant for an in-house Skilled Nursing Facility. You will provide physical therapy services under the direction of a licensed Physical Therapist, assist in screenings, and help develop treatment plans to improve residents' mobility and quality of life.

This PRN role emphasizes compassionate care, adherence to CMS expectations in NetHealth EMR, and collaboration with the therapy team to support resident goals.

Qualifications

• Assists in planning, organizing, and directing PT services under supervision.
• Active PTA license in state(s) of practice required.
• Associate degree in physical therapy from an accredited program.
• Familiarity with CMS expectations and documentation in EMR systems.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist.
• Assist in establishing target goals and treatment plans.
• Conduct individual and group treatments including strength, mobility, gait, balance, and ROM activities.
• Monitor residents’ response to intervention and consult with the Physical Therapist as needed.
• Support documentation and schedules in coordination with the therapy team.
