Job summary

Evolve Therapy Services is a therapy management organization seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Newport, TN. The PRN position supports licensed therapists and ensures resident care quality.

The PTA will conduct screenings, participate in treatment plans, and provide both individual and group therapy sessions under supervision, while maintaining CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide PT services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in developing treatment plans with short- and long-term goals.
• Assist in selecting PT techniques and activities per treatment plan.
• Document compliant and medically necessary notes in NetHealth EMR.
