Job summary

Evolve Therapy Services in Waverly, NE is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility, PRN. The role supports the Physical Therapist, conducts screenings, and helps implement treatment plans focused on mobility, balance, gait, and strength to maximize resident independence.

The position requires an Associate's degree in physical therapy, an active PTA license, and commitment to ongoing education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and in developing short- and long-term treatment plans with the Physical Therapist.
• Carry out treatment plans including mobility, gait, ROM, strength, balance, and modalities as directed.
• Assist in selecting techniques/media and adapt as needed per resident progress.
• Train residents, families, and staff in use of equipment and therapy processes.
• Document CMS-compliant progress notes and maintain NetHealth EMR records.
• Participate in Quality Assurance activities and coordinate resident schedules with the PT.
