Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Waverly, NE. You will provide PT services under the supervision of a PT, conduct screenings, and help develop treatment plans to improve residents’ function and quality of life.

Responsibilities include documenting CMS-compliant notes in NetHealth EMR, coordinating with the therapy team, and supporting residents and families in their rehabilitation journeys.

Qualifications

• Requires an Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under the supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Assist in evaluation and in developing treatment plans and goals.
• Implement treatment plans including strength, mobility, gait, balance, and modalities.
• Document compliant and medically necessary CMS-relevant notes in NetHealth EMR.
