Evolve Therapy Services in Sutherland, NE is seeking a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility. This PRN role works with a Physical Therapist to deliver resident-centered therapy.

Duties include screenings, preparing evaluations, setting short- and long-term goals, providing individual and group sessions, and documenting care in NetHealth EMR per CMS guidelines.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• CMS documentation requirements in NetHealth EMR systems.

Responsibilities

• Provide physical therapy services under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine treatment needs.
• Assist in evaluations and develop treatment plans with short- and long-term goals.
• Deliver individual and group therapy addressing mobility, strength, balance, and gait.
• Document interventions in NetHealth EMR per CMS expectations.
