Job summary

Evolve Therapy Services in Omaha, NE seeks a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role supports the Physical Therapist with direct resident care, screenings, and treatment planning to improve mobility, strength, and function.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist in establishing target goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan, including mobility, strength, balance, gait, and ROM activities.
• Monitor residents' response to intervention and consult with the Physical Therapist for modifications.
• Assist in scheduling and coordinating patient care with the team.
• Train residents and staff in the use of equipment and participate in education.
