Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant to join our in-house Skilled Nursing Facility program in Sutherland, NE on a PRN basis. The PTA will work under the Physical Therapist to deliver resident-centered therapy and support a high standard of care.

The role involves screenings, assisting in evaluations and treatment plan progression, plus documentation in NetHealth EMR.

Qualifications

• Requires an Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice required.
• Continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluation and in identifying goals and treatment plans with the Physical Therapist.
• Deliver individual and group therapy per the treatment plan (strength, mobility, gait, ROM, etc.).
• Assist in selecting techniques/media and adapt as indicated.
• Monitor resident responses and modify therapy as advised by the PT.
• Coordinate resident schedules with the PT and other team members.
• Maintain compliant and medically necessary CMS documentation.

Education

Associate's degree in Physical Therapy
